Becoming a patient
To become a Coastal Health & Wellness patient, please complete the Coastal Health & Wellness Registration Form. For information on what documents to bring to the clinic for financial screening see the Coastal Health & Wellness Registration Instructions.
Who do we serve
Our mission is to provide access to high quality primary care to any and all individuals in need.
Insurance accepted
We welcome Medicaid, Medicare and private insurance.

Uninsured programs
Discounted health services for the indigent and uninsured up to 200% Federal Poverty Rate - if you are uninsured or if the Coastal Health & Wellness care you need is not covered by your health plan and your income is less than 200% of the federal poverty rate, you may be eligible for discounted services at Coastal Health & Wellness. The attached federal poverty scale outlines income limits by family size. To be considered for this program, you complete the Coastal Health & Wellness Registration Form and you must be financially screened by Patient Services Staff in the clinics. Discounts are provided as part of the Coastal Health & Wellness federal grant.
Title V Program for uninsured children in households below 185%
If your child is 0 – 21 years of age and not eligible for Medicaid, he/she may be eligible for the Title V program which pays for the cost of well and sick child visits and certain dental services. Upon financial screening of your household, a determination can be made. Remember to bring all the documents needed for financial screening. Women's Health Program
If eligible, this program pays for an annual family planning exam and certain birth control pharmaceuticals. For more information on eligibility and requirements, see attached flyer.
