Available days and times: Tuesday, Wednesday, or Thursday at 8:30 a.m.                                                                        

2. Items to bring

1. State identification card, driver license or state-approved documents. 点击 英语 / 西班牙语 for required documents.
2. $47 (money order) for the state fees
3. $7 (cash, credit or debit) for the notary fees

The AOP appointment may take up to two hours

4. State process

The state can take anywhere from three to six months to have the father added to the birth certificate. You will receive the new birth certificate through the mail. For further information contact DSHS - 1.888.963.7111.

NOTE: Both parents must present a valid identification or driver license at the time of the appointment and are required to be on time or you’ll have to reschedule your appointment.
